When a removable denture will not stay in: three fixes — locators, a bar, or All-on
Why a removable denture holds poorly, especially on the lower jaw, and three ways to fix it: a removable denture on two implants with locators (400,000 ₸), a removable one on a bar on four (500,000 ₸), or a fixed All-on-4/5 (from 1,200,000 ₸). Tursunov Clinic case studies, Astana.
Key points
- A removable denture stays in place by suction against the palate and the gum. The lower jaw has no palate, and the gum shrinks over time — there is nothing for the denture to grip. This is not "a bad denture" or "you haven't adjusted" — it is anatomy.
- There is one fix: give the denture support — implants. There are three options, from budget-friendly to definitive.
- Two implants with locators (400,000 ₸): the denture stays removable but snaps onto two implants and no longer shifts.
- Four implants with a bar (500,000 ₸): the removable denture sits on a rigid bar — it holds like a fixed one, and comes off for cleaning.
- All-on-4/5 (from 1,200,000 ₸): a fixed denture on 4–5 implants, feeling like your own teeth.
Why the denture does not stay in
A full removable denture on the upper jaw is held by the palate: a wide surface, suction, the denture "sticks." On the lower jaw the surface is narrow, the tongue sits in the middle, and with every year without teeth the ridge bone shrinks further — the denture sits on gum that gets flatter and more mobile. The result: it lifts while talking, shifts while chewing, and rubs. Adhesive creams help for hours, not as a fix.
That is exactly what both of the clinic's cases were about: "the problem wasn't how the denture was cared for — it was the lack of support."
Fix 1: two implants with locators — 400,000 ₸
Two implants are placed in the canine area of the lower jaw (less often the upper). Small "snaps" (locator or ball attachments) sit on them, and matching housings sit in the denture. The denture clips in like a button on clothing.
What it gives you: the denture no longer lifts or shifts; you can chew with confidence; it comes off at night for cleaning.
For whom: patients 60+ with a full lower-jaw clearance, where budget matters and major surgery is not wanted. This is the global standard for minimal treatment of an edentulous lower jaw.
Drawbacks: the denture is still removable and rests on the gum at the back; the housings (rubber inserts) wear out and are replaced every year or two.
Timeline: the implants are placed in one visit, and the denture is fitted after integration (2–3 months); an existing denture can often be adapted.
Fix 2: a bar on four implants — 500,000 ₸
Four implants are joined by a titanium bar, and the denture sits on top of it. It is still removable (implant-retained), but it holds onto the bar rigidly rather than resting on the gum.
What it gives you: retention close to that of a fixed denture, the gum is unloaded, and you can chew harder food.
For whom: when locators alone are not enough (heavily resorbed bone, weak gum) and a fixed denture is out of budget or not suitable for hygiene reasons.
Drawbacks: more expensive than locators, needs four implants; the denture is bulkier.
Fix 3: All-on-4 / All-on-5 — from 1,200,000 ₸
A fixed denture on four or five implants, screw-retained. The patient never removes it — it is cleaned like natural teeth; only a doctor can remove it at a check-up.
What it gives you: the feel of natural teeth, clear speech, no "removable" feeling at all.
For whom: those who want to close the question for good and are ready for the price: All-on-4 Izen — 1,200,000 ₸, All-on-5 Izen — 1,500,000 ₸ with a temporary denture; the permanent denture is billed separately (acrylic 800,000–900,000, a titanium bar 1,500,000–1,600,000).
A case: a patient whose lower denture would not stay in place received four implants and a fixed denture on a titanium bar. Another patient, who came with a denture from another clinic, received five implants on the upper jaw and a bar-retained denture within days; the lower jaw kept its natural teeth.
How it is chosen
The prosthodontist and surgeon look at: bone volume (a CBCT scan, 11,000 ₸), the state of the opposite jaw (sometimes it still has natural teeth, and the two jaws get different solutions — as in the case with All-on-5 on top and a thermoplastic partial denture below), overall health, budget, and readiness for aftercare. The path is often staged: locators first, then All-on years later on the same or new implants.
Implants — 2 — 4 — 4–5
Denture — removable — implant-retained removable — fixed
Price — 400,000 ₸ — 500,000 ₸ — from 1,200,000 ₸ + permanent denture
Care — remove at night — remove for cleaning — like natural teeth + a water flosser

Frequently asked questions
Can my old denture be converted for locators?
Often yes, if it is in good condition; the prosthodontist will check.
I'm 70 — is it too late?
Two implants with locators is the gentlest surgery in implant dentistry, done even at 80. There is a separate article on implants after 60.
How long between the implants and the denture?
2–3 months for integration; with All-on, a temporary denture is fitted within the first few days.
What about my upper denture, if it already holds fine?
Nothing needs to change there: only the jaw with the problem is treated.
